Your consent forms will be sent to you in a separate email labeled under "sign now" with your email address that you made with your appointment details. Your consent forms will need to be completed 48 hours before the scheduled appointment. 

Unfortunately, we do not issue refunds. To guarantee your appointment, a deposit is required. You will pay for the rest of the procedure in person. Your deposit is non refundable. If have to reschedule or cancel your appointment, you will have to create a new appointment and deposit. Thank you for understanding. 

 After you have scheduled your appointment, we will email or text over you consent forms and parking directions before your arrival. If you do not confirm your appointment 48 hours, your appointment will be cancelled. Your deposit will be non refundable. You will have to reschedule a new appointment. Thank you for your cooperation. 

Upon arrival of the day of your procedure, your discount or promo will be included upon your purchase. We have exciting new offers as the seasons change!
